You can download our fillable PDF Form (use Adobe Acrobat) pictured below and email or fax it to us at 877-489-3011. You enter the Item #, Quantity and Price for each, and it will calculate the total.
Please Note:
The minimum order shipped is $15.00.
If you choose the PayPal option, be sure to include the email address you want us to send the PayPal RFP [Request for Payment] notice to. Shipments will not be released until the PayPal RFP has cleared.
We also ship Worldwide. Pricing is in USD.
Our normal business hours are 8:30 AM to 4:30 PM- EST. Monday through Friday but we do check our email on weekends.
Return Policy: Even though each item is inspected for quality before it is packaged, should you receive a defective piece, we offer a 30 day refund or replacement policy. We will pay return ground shipping costs, but please call us at 877-489-3011 to resolve any issues beforehand.
